1. Who we are and what this covers
Regen Wellness ("we," "us") is a functional medicine clinic at 5510 Abrams Rd, Suite 126, Dallas, TX 75214. This policy explains how we handle information collected through regenivwellness.com, our online forms and tools, and the emails and text messages we send.
It does not cover information in your medical record, which is governed by HIPAA and our Notice of Privacy Practices, or third-party websites we link to, such as our patient portal, online scheduling and supplement store. Those sites have their own privacy policies.
2. Information we collect
Information you give us
- Contact details you enter in forms, such as your name, email address and phone number.
- What you tell us when you request an info call, ask a question, request pricing or contact us.
- Which of our online tools you used, as described in section 3.
Information collected automatically
- Your IP address, browser and device type, the pages you visit, and how you arrived at our site.
- Cookies and similar technologies, described in section 7.
- Basic call information when you call a phone number shown on our website, described in section 7.
3. Our online health tools
Some pages include educational tools, such as Decode My Labs. Here is exactly how they handle your information:
- Lab values and symptoms stay on your device. The numbers you enter and the symptoms you check are calculated in your own browser. They are not sent to us or saved by the page, and they're cleared when you leave.
- What we receive, only if you agree. If you submit the form to see your results, we receive only what you enter in the form (such as your name and email) and which version of the tool you used (with or without lab results). You agree to this with a checkbox before submitting.
- How we use it. To send you your lab guide and follow-up emails, and to respond if you ask to speak with us.
- Not medical advice. The tools are educational. Using them does not make you a patient or create a provider-patient relationship.
4. How we use information
- To respond to your requests and schedule info calls and appointments.
- To provide care and services if you become a patient.
- To send emails about our services, events and health education, if you've given us your email address.
- To understand how our website is used and improve it.
- To measure which advertising and marketing brings people to us, without sharing health information with ad platforms (see section 7).
- To protect our clinic, patients and website, and to meet legal requirements.
5. Patient health information and HIPAA
When you become a patient, information in your medical record, including lab results, diagnoses and treatment, is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). We use and share it only as described in our Notice of Privacy Practices, which also explains your rights to access and amend your records.
Please don't send medical details through our general website forms. Use the patient portal or call us at 214-326-0033.
6. Emails and text messages
Email. If you give us your email address, for example to get a lab guide, we may send you related emails and occasional updates. Every marketing email includes an unsubscribe link.
Text messages. We do not send marketing text messages. Patients may receive texts through our patient portal, such as appointment reminders and portal notifications. Message and data rates may apply. Reply STOP to opt out, or ask our front desk to change how you're contacted. We do not share or sell your mobile number or text-messaging consent with third parties for their marketing.
7. Cookies, analytics and call tracking
- Cookies help our website work and remember your preferences. You can block or delete cookies in your browser settings, though some features may not work.
- Analytics. We use Google Analytics and our website platform's reporting (Marketing 360) to understand how visitors use our site, such as which pages are viewed and how people find us. You can opt out of Google Analytics with the Google Analytics opt-out browser add-on.
- Call tracking. Some phone numbers on our website are tracking numbers that forward to our clinic. They help us see which pages and ads lead to calls. Our phone system may record the calling number, date, time and length of the call. Calls to tracking numbers shown in our advertising may be recorded for quality, training and to measure our ads.
- Advertising. We may advertise on Google, Meta (Facebook and Instagram) and Microsoft Advertising (Bing). These platforms may use cookies or similar tools on our site to measure our ads and show them to people who may be interested. We do not send what you enter in our online tools, including lab values and symptoms, or any medical information to advertising platforms. You can manage ad personalization in your Google, Meta and Microsoft ad settings, or opt out of interest-based ads from many companies at optout.networkadvertising.org.
9. How long we keep information
We keep contact and marketing information while you're on our email list or in contact with us, and delete leads from our online tools after 12 months if you don't become a patient. Medical records are kept as long as Texas law requires. You can ask us to delete your information at any time (see section 10).
10. Your choices and rights
- Unsubscribe from emails with the link in any email, or reply STOP to texts.
- Access, correct or delete the personal information we hold about you from our website and marketing, by contacting us (section 14).
- Depending on where you live, including Texas, you may have additional rights, such as confirming whether we process your information, getting a copy of it, and appealing a decision we make about your request. Contact us to make a request. We will respond within the time the law requires and won't treat you differently for using these rights.
- Patients can request access to or amendment of their medical records as described in our Notice of Privacy Practices.
11. Security
We use reasonable administrative, technical and physical safeguards to protect information. No website or email system is completely secure, so please avoid sending sensitive medical details through general web forms or email.
12. Children
Our website and online tools are intended for adults 18 and older. We do not knowingly collect personal information from children under 13 through our website. If you believe a child has given us information, contact us and we'll delete it.
13. Changes to this policy
We may update this policy from time to time. We'll post changes on this page and update the date at the top. If we make a significant change, we'll post a notice on our website or email you.
