Find Out Why the Weight Won't Come Off
Testing and care for insulin resistance, thyroid problems, menopause, low testosterone and other causes of weight gain, from our functional medicine team in East Dallas.
Why Can't I Lose Weight?
When weight won't come off despite eating well and exercising, there's often a medical reason underneath. The most common are insulin resistance, an underactive thyroid, the menopause transition, low testosterone, high stress and poor sleep, and medications such as steroids and some antidepressants. These can increase hunger, change where your body stores fat and make weight harder to lose.
At Regen Wellness in Dallas, a licensed provider tests for these causes and builds a plan around what we find, from nutrition and hormone care to prescription weight-loss medicine when it's appropriate.
Common Causes of Hormonal Weight Gain
Most people have more than one of these at the same time. Finding them is the first step.
Insulin Resistance
Your body needs more insulin to manage blood sugar, which makes it easier to store fat, especially around the middle.
Underactive Thyroid
A slow thyroid can cause weight gain, fatigue and feeling cold.
Thyroid & Hashimoto's →Perimenopause & Menopause
Falling estrogen shifts fat toward the belly and can make weight harder to manage.
Menopause treatment →Low Testosterone
In men, low testosterone is linked to more body fat and less muscle.
Low T treatment →Stress & Sleep
Short or poor sleep and long-term stress raise hunger and cravings and affect blood sugar.
Medications
Steroids, some antidepressants, diabetes medicines and others can cause weight gain. Don't stop them on your own.

Insulin Resistance: Symptoms and Treatment
Insulin resistance is very common and often goes undiagnosed for years, because blood sugar can stay normal while insulin levels climb. Left alone, it can lead to prediabetes and type 2 diabetes.
Common signs: weight gain around the waist, cravings or energy crashes after meals, darker, velvety skin on the neck or underarms, skin tags, high triglycerides or blood pressure, and, in women, irregular periods or PCOS.
Treatment starts with nutrition, strength training, sleep and stress, which all improve how your body uses insulin. When those aren't enough, your provider may discuss medications such as metformin or a GLP-1 medicine.
Know Your Numbers
Hemoglobin A1c: 5.7–6.4% is prediabetes. 6.5% or higher on repeat testing is diabetes.
Fasting glucose: 100–125 mg/dL is prediabetes.
Fasting insulin: no single official cutoff, but higher levels can show insulin resistance before glucose rises. We read it alongside your other results.
Metabolic Testing We Use
Your provider chooses tests based on your history and symptoms, so not everyone needs every test.
| Area | What we check | Why it matters |
|---|---|---|
| Blood sugar & insulin | Fasting glucose, A1c and fasting insulin | Finds insulin resistance and prediabetes early |
| Thyroid | TSH, free T4, free T3 and antibodies | An underactive thyroid slows metabolism |
| Sex hormones | Estradiol, progesterone and testosterone, based on your age and symptoms | Menopause and low testosterone change body composition |
| Cholesterol & liver | Lipid panel and liver enzymes | Shows metabolic strain and fatty liver risk |
| Cortisol | Properly timed cortisol tests, when your symptoms suggest a problem | Rules out conditions such as Cushing's syndrome |
| Real-time glucose | Continuous glucose monitoring (CGM) | Shows how your own blood sugar responds to meals, sleep and exercise |

What About "Cortisol Belly"?
"Cortisol belly" is a social media term for belly fat blamed on stress. There's some truth to it: long-term stress and poor sleep raise cortisol, which can increase appetite and fat stored around the middle. But for most people, stress is one factor among several, alongside insulin resistance, hormones, sleep and diet. Cortisol supplements and "cortisol detox" products sold online aren't proven to help.
Truly high cortisol from a medical condition, such as Cushing's syndrome, is rare but serious. If you have signs such as a rounder face, wide purple stretch marks, easy bruising or muscle weakness, we'll test properly and refer you to an endocrinologist.
Our Approach to Weight Gain and Metabolism
Every plan is personalized to your results. Most draw on these steps:
- Treat the cause. Thyroid treatment, hormone therapy for menopause or low testosterone, or medication for insulin resistance when your labs call for it.
- Nutrition you can keep. Enough protein and fiber, fewer refined carbohydrates, and a plan built around how you actually eat, not a crash diet.
- Strength and movement. Building muscle improves how your body uses insulin.
- Sleep and stress. Practical changes that lower cravings and support blood sugar.
- Supplements only with a reason. Chosen from your results. Your provider will recommend specific products, and you can find the practitioner-grade options we use in the hormones and metabolism collection at Regen Wellness Vitamins, our supplement store in the same suite.
- Prescription weight-loss medicine when appropriate. If you qualify, GLP-1 medicines such as semaglutide or tirzepatide are available through our medical weight loss program.

What hormones cause weight gain?
Insulin, thyroid hormone, estrogen, testosterone and cortisol all affect weight. Insulin resistance, an underactive thyroid, menopause, low testosterone and long-term stress are the most common hormone-related causes.
What are the symptoms of insulin resistance?
Weight gain around the waist, cravings and energy crashes after meals, darker skin patches on the neck or underarms, skin tags, and high triglycerides or blood pressure. Many people have no obvious symptoms, which is why testing matters.
Can insulin resistance be reversed?
Often it can be greatly improved. Weight loss, strength training, better sleep and diet changes improve how your body responds to insulin, and medication can help when needed. Results vary, and some people need ongoing treatment.
Why am I gaining weight during menopause?
Falling estrogen shifts fat toward the belly, muscle mass declines with age, and sleep often gets worse. Hormone therapy may help some symptoms, and strength training and nutrition matter more than ever. We look at all of it together.
Is a slow metabolism real?
Metabolism varies less between people than most think, but it does slow with muscle loss, an underactive thyroid and long periods of dieting. Building muscle and treating thyroid problems are the most reliable ways to support it.
Do I need a GLP-1 medicine?
Not everyone does. We start by finding the cause of your weight gain. If you qualify and lifestyle changes aren't enough, medicines such as semaglutide or tirzepatide may be an option through our medical weight loss program.
